In the 1800s it was beginning to be recognized that rehabilitation after an injury was important. Orthopedics started to become a separate field from general surgery. A famous British Surgeon, Hugh Owen Thomas, created specialty splints that were cheap and best for injuries that were being rehabilitated. By 1883, mechanics and surgeons separated due to class issues. This created two different areas that shaped the way braces were being created and distributed. Around 1888, F. Gustav Ernst, a dedicated mechanic, released a book illustrating upper body splints. In 1899, orthopedic surgeon Alessandro Codivilla followed suit and published a book explaining the importance of using surgical procedures to set up better results using splints. [9] See also [ edit ] Buddy splints: These involve two fingers taped together. People use buddy splints when they have a strained finger — for example, due to a jamming injury. This type of splint is not suitable for fractured fingers. A splint is loosely defined as an external device used toimmobilize an injury or joint and is most often made out of plaster or fiberglass.Varioussplinting techniques may be employed, dictated by the location of the injury and the position of immobilization needed.Splinting aims to correct and restore the anatomic length, rotation, and angulation of a specific injury. [2] [1] [3] No specific contraindications to splinting exist. However, certain injuries and patient-specific co-morbidities require special attention: Followingfracture splinting, follow-up care should be coordinated for the patient to ensure improving clinical status. For injuries that do not require specialty care or operative fixation, often this coordination occurs between emergencypractitioners or first responders and primary carepractitioners. This is particularlyrelevant in the case of pediatric forearm fractures, where most patients receive follow-up care with primary carepractitioners and not orthopedic specialists. [17]

Your provider will tell you which type of splint you’ll need. They’ll tell you where you can buy one and show you how to use it. Splints are typically pre-made devices that aren’t custom-fitted to your body. Your provider might recommend a custom-fitted splint, but it’s less common. Avoid placing pressure or weight on the cast. If you have a leg injury and have a walking cast, make sure that the cast has hardened completely before you attempt to walk on it. Wearing a splint is a common treatment for lots of injuries. You may also need one to support your body if you have a health condition that causes weakness or affects your strength. Percutaneous means "through the skin". As with open surgery, the surgeon will inject your hand with a local anaesthetic. However, instead of making an incision, a needle will be inserted into the base of the affected finger and used to slice through the ligament to get to the tendon.Upper extremity splints crossing the wrist should maintain neutral wrist dorsiflexion. In pediatric patients with supracondylar elbow fractures, the arm should be cast or splinted in a long arm cast with the elbow flexedbetween 60 and 90 degrees. The elbowshould never be splinted while flexed more than 90 degreesdue to the increased risk of Volkman ischemic contracture. [6] [7]

Trigger finger release surgery is a safe procedure. However, as with any type of surgery, there are some risks. Complications are rare, but could include:The neurovascular status of the injured extremity must be assessed before and after splint placement. Additionally, excess pressure on the soft tissues may decrease the blood flow to the skin surface, causing irritation and necrosis; this is particularly important in areas with bony prominences, such as the elbow, knee, and calcaneus. Additional layers of protection applied during the splinting process are of great importance in these regions. For example, if you’re splinting a forearm, place the rigid support item under the forearm. Then, tie or tape it to the arm just below the wrist and above the elbow. A steroid injection can be used to reduce swelling. In cases of trigger finger, liquid corticosteroids are injected into the base of the affected finger or thumb.
